Original document link: http://district.sanjuan.edu/?p=374 (accessable from Intranet only) San Juan Unified School District and the San Juan Unified Wellness Committee are pleased to announce FREE Kaiser sponsored health risk assessments. The health risk assessments will be held at the locations below and are open to San Juan Unified employees only. Please note that this offer is open to all District employees at any of the locations listed below (Kaiser, PacifiCare and employees without District health plan coverage). This is being offered as another component of our Employee Wellness Program. Details of the program are located on our website EmployeeWellness .
The health risk assessment will focus on “knowing your numbers.” Each participant will complete a lifestyle assessment questionnaire, receive Cholesterol (Total Cholesterol, HDL & Ratio), Diabetes (Blood Sugar Level), and Blood Pressure Screening. Participants will receive same day screening results, and in 10 days a Personal Wellness Profile Report will be mailed to their home address. The report is a detailed personal and confidential educational assessment designed to help participants evaluate their current lifestyle, identify health risks, and decide where and how to make improvements. The results of the health risk assessment are completely confidential and will not be shared with employee’s health care providers or the school district. Posters (see attachment) describing the event will be placed at each District site and employees will receive an email notice of this program. As an additional incentive, Kaiser has donated one Wii Fit. The winner will be selected from the completed health risk assessments. For taking the health risk assessment, all employees will also receive a PacifiCare sponsored pedometer. This pedometer will be a useful tool in our upcoming Walking Program to be launched this spring. Health Risk Assessments Poster DATES AND LOCATIONS FOR
